I filled my tank last night. I do not have a ventectomy. I put the nozzle in and locked it on fast. I left my hand on the handle. "Dang this is taking a lot and I have to pee" I smell fuel and there is a puddle on the ground and diesel is spilling out the filler neck. Why did the fuel pump not click off? At the time a tanker was delivering fuel. I don't know if it was diesel. A quart or two of diesel on the ground.

Is this a fuel station problem or a fuel tank problem?

Pump issue
Not uncommon of an issue. You got a bad dice roll.
There is basically an air vent spring that stops the flow. When it fails it keeps pumping.
Complain to the station next time and get some $ back for the fuel you paid for that didnt go in your tank.
