usually a cause to such a failure is from a defect within the rod. I'd closely inspect the crank where the rod rode on just in case for any scoring. I wish i could find the picture of the cummins failure. Crank snapped, wedged into the block tearing off cylinder 5 and 6 as well as the converter...
