Massachusetts medical exemption for window tinting

Anyone have any experience doing this? I've had significant issues with my eyes for years and suffer sensitivity to light (among other things) and have always had my vehicles tinted to help withy his. (No it isn't as simple wearing sunglasses). Anyone in mass go through this process?

No experience w/ this item. How dark is needed to help? I had a 35% tint performed in Billerica; they will only use a darker formula w/ cash.

Looks like all you need to do is fill out the form

and give it to your doctor to fill out the form.

Doesn't say anything about submission to the RMV.
